How to Send

  1. Open the patient's profile
  2. Click Send Form
  3. Select the form template
  4. Choose delivery method: Email or WhatsApp
  5. Click Send

Patient Access

  • •Works without login (token-based authentication)
  • •Is valid for a configurable period (24-168 hours)
  • •Can be accessed from any device
  • •Supports save & resume functionality

Tracking

  • •Sent, Viewed, Completed, or Expired
  • •Completion date and time
  • •Notification sent to staff when patient completes form
